Hedi Marnhout, Phantasm IV: Oblivion, 1997.     Auteur Don Coscarelli wanted Jennifer-the-role to be played by Jennifer-the-wife of his hero (A Michael Baldwin –  as opposed to The Michael Baldwin?).  She passed. Apparently her  cameo as a nurse  in Phantasm III : Lord of the Dead, in 1992, was enough for her.  (And everyone else; III and IV  went straight to video).  Jennifer  agreed, however, to voice the fortune teller. Coscarelli  remains one of the few franchise creators to admit that IV was made – shock! horror! –  “just to make money out of the series.”
